Family Support Service

Definition:

This service includes intensive services, counseling, training and support to individuals with developmental disabilities, their natural families and service providers.

Service Goals

This service enables an individual to:
1. Live in a community environment such as independent living, natural home or other home-like living arrangement.
2. Continue to remain an integral member of his or her family.
3. Achieve social inclusion in the natural environment or community.

Target Population
Adults and children of all ages, birth to death, who reside in the community with their natural families or in homes of their own.

Typical Setting

Support services are provided in the individual’s home.

Typical Activities
The Client and Family Support Program provides linkage to an array of services that include, but are not limited to:

1. Psychological, Speech, and Psychiatric Evaluations
2. Educational and Vocational Services
3. Medical / Physical Assessments
4. Supportive, Individual and Family Counseling
5. Respite
6. Housing
7. Parent Training
8. Substance Abuse Counseling
9. Behavior Management Assessment
10. Transportation
11. Occupational and Physical Therapy
12. Guardianship
13. Assistive Technology
14. Intervention, i.e. Domestic Violence

Service Boundaries

Roosevelt Road (north) to 75th St. (south); the Dan Ryan Expressway (east) and west to the city limits (Narragansett).
